Clinical History: 18 year old male with complex, enhancing mass in the posterior aspect of the left kidney found incidentally on imaging done post-traumatic fall evaluation. Mom has been diagnosed with cerebrovascular malformations.

Histology: Figure A shows moderately to markedly irregular nuclear contours with visible nucleoli (Fuhrman grade 3). Special IHC show tumor cells with diffuse cytoplasmic and cell membrane staining for e-cadherin, patchy staining for RCC. Colloidal iron staining is also negative in the tumor cells. This tumor antigenic profile is not characteristic of any of the classified renal cell carcinomas, with the closest possibility of a papillary type with mixed features of clear cells and oncocytic cells.
