Clinical History: 72-year-old man with history of von Hippel-Lindau disease and thyroid cancer, with bilateral renal cysts and chronic kidney disease. Patient underwent right partial nephrectomy to remove the lesion.

Histology: The sections show papillary renal carcinoma with patchy clear cell component. There is variegated appearance and discrete papillary fronds lined by neaoplastic epithelial cells. Cells are cuboidal to low columnar, basophilic with scant cytoplasm. Figure C shows presence of foamy macrophages. Special stains in Figure G and H show tumor cells strongly positive for CK7 in the areas with papillary configuration and negative in the areas of clear cell differentiation, focal reactivity for CA-IX in the areas of clear cell differentiation.
