Clinical History: A 38 year old female with known history of lupus erythematosus. She was previously treated for lupus nephritis. Creatinine is normal but proteinuria has been rising over past 3 months.

Histology: The glomeruli show variable increases in mesangial matrix and cellularity.  Figures C and D show thickened peripheral capillary loops with spike-and-dome formations and focal segmental basement membrane reduplication. The tubules lie back-to-back without significant interstitial fibrosis or inflammation (Figure B).
